Congenital Left Atrial Aneurysms
67-year-old man with atrial fibrillation was referred for pulmonary vein ablation. Preprocedural contrast-enhanced CT revealed two 4–5-cm well-circumscribed outpouchings with broad necks arising from the left lateral and posterior walls of the left atrium (Figs 1, 2). The left atrial appendage was also aneurysmal without any thrombus (Figs 1B, 2B). Absence of coexisting mitral valve disease, wide aneurysmal necks, and intrapericardial location were compatible with a congenital etiology. Pulmonary vein isolation with wide antral ablation was successfully performed after carefully avoiding the aneurysmal portions.
