Clinical history & presentation:
22-month-old boy presenting with high-grade fever, with maximum temperature of 104 degrees F. Previous chest radiograph obtained few months ago (not shown) was normal.
Physical exam:
On physical exam there are decreased breath sounds in right upper lung. No nasal flaring, grunting, wheeze or crackles were present.
Other diagnostic testing:
White blood cell count - 37.1 thou/mcL ( normal 5.0-15.0 thou/mcL).
Frontal chest radiograph shows a round opacity in the right upper lobe (white arrows).
Lateral chest radiograph shows a focal consolidation with well-defined borders in the right upper lobe (white arrows) abutting the major fissure.
